To activate your UCard, follow these steps:

1. Visit the Activation Website

   • Open your web browser and go to activate.uhc.com.

2. Enter Your UCard Information

   • Locate your UCard and input the required details:
     • Your Card Number (found on the front of your card).
     • Your Member ID (if requested).

3. Verify Your Identity

   • Provide the requested personal information, such as:
     • Your Date of Birth.
     • The Last Four Digits of Your Social Security Number (SSN).
     • Any additional prompts for verification.

4. Create or Log Into Your Online Account

   • If you don’t already have a UnitedHealthcare online account:
     • Create one by entering your email, setting a username and password, and establishing security questions.
   • If you already have an account, log in to proceed.

5. Complete the Activation

   • Confirm that all details are correct and submit the activation request.
   • You’ll see a confirmation message once your UCard is successfully activated.
